• If you (the owner) authorize an agent to file a return on your behalf or to receive confidential information, you will need to submit this application in place of a letter of authorization.
• If you do not wish to submit this application, view our OCPA Requirements For TPP Letters Of Authorization .
• If you are not the owner of the property but a corporate officer, please provide your title.
• The tax year(s) for which the authorization is granted must be stated.
