• Income and Expense Statements as of December 31, 2025 and for the two years prior (2023 and 2024)
• Balance Sheets as of December 31, 2025 and for the two years prior (2023 and 2024)

Submit your completed Income and Expense Survey form and documentation no later than April 17, 2026.
